In consideration of my choosing for my child to take part voluntarily in the Sycamore at The Stadium Baseball Camp, I acknowledge and agree that participation presumes a possible risk of exposure to and illness from contagious diseases, including, but not limited to, Covid-19, MRSA, and influenza. While particular rules and physical disciplines may reduce this risk, the risk does exist, and I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown. 

I also assume any risk of accidental injury that my child could experience during the Sycamore at The Stadium Baseball Camp. In the event of any such illness or injury, I agree fully to release, indemnify, and hold harmless The Sycamore Church as well as any coaches, volunteers and participants of the Sycamore at The Stadium Baseball Camp, from any and all liability, claims, and/or litigation that myself or my child may have which are directly or indirectly related to my child’s participation while at The Sycamore at The Stadium Baseball Camp. 

I further understand that this document supplements, and in no way eliminates, all other documents, waivers and releases signed on behalf of my child for his/her participation in The Sycamore at The Stadium Baseball Camp.
